In compliance with Spain´s Data Protection Organic Act 15/1999 dated 13 December, you are advised that the personal information you give us will be stored in GIPUZKOA MEDICAL ASSOCIATION files whose sole function is the fulfilment of the Association´s purposes as described in national and regional professional associations legislation and especially in the byelaws of the corporation. These include arranging within its remit the medical activity of its members, ensuring professional ethics and dignity, representing the profession and its members in all kinds of processes and procedures affecting the professional interests of any member, requesting expert reports, ensuring the rights of individuals, exercising disciplinary powers and managing its members´ private interests.
To that end we are asking for your consent to process your personal information and release it to private organisations with which the Association reaches agreements that are beneficial for its members.
You may exercise your right to access, change, delete and oppose the processing of your personal information as recognised in the aforementioned Act 15/1999 by writing to ILTRE COLEGIO DE MÉDICOS DE GIPUZKOA, PASEO DE FRANCIA Nº 12, 20012 SAN SEBASTIAN, enclosing a photocopy of your ID, or by using the forms available at the above address.
