You will need to fill in a document applying to cancel your membership of the Gipuzkoa Medical Association due to stopping being a doctor or moving to another association. You must also submit the original of your paid bill for the last quarter and return your collegiate card
Documents to be submitted:
- Membership cancellation application form
- Original bill for the last quarter paid
- Return of the Collegiate Card
- For the cancellation to take effect you must be up-to-date with payment of both Association and professional liability insurance fees
